Adult Services Case Management 

This service is offered by the Centre County Office of Adult Services to provide medium- and long-term case management services to individuals/families who need assistance with securing and maintaining basic needs (housing, food, utilities, medical, etc.), navigating the human services system for appropriate resources, and following client-centered goals and service plans to improve self-sufficiency.

Basic Needs Case Management

This service is offered by Centre Helps to provide short-term case management to help eligible residents resolve immediate financial crisis, provide in-depth resource referral, and offer both budgeting solutions and skills.

Basic Needs Medical Case Management

This service is offered by Centre Volunteers in Medicine (CVIM) provides short-term case management to CVIM clients and community members who are uninsured and/or are in need of health care/navigation and enrollment support. This case management service can also provide in-depth resource referral and guidance for resolving immediate financial crisis.

Service Planning Case Management

This service offered by Housing Transitions provides long-term case management for eligible residents with physical disability and who may need or currently receive homemaker services.
